Session 7: Seam Summer Science Institute
Activity 4:  Creating a Form and a Confirmation Page

(40 minutes)

Open Front Page and select 'File...New...Form Page Wizard'. 

Using the handout provided, create a Warm Up form with:

• Space for personal information (name, student ID, and a generic location for nickname)
• Three different styles of questions (ie: essay, multiple choice, 'any of several options', etc)
• Fill in the content of the questions only if  you have time - you can add details after the main form is created. (Save as warmup.htm)
• Using Form properties, set form up to be sent to you as an e-mail or as a file (save as  warmupresults.htm)
• Using Form properties, set form up to generate a confirmation page (save as warmupconfirm.htm)

Create a confirmation page 7/26/03: We will NOT be doing this in our Seam Summer Science Institute Workshop! The Center for Teaching and Learning will contact us when they have written the correct asp script for teh forms we will be using !

• Start with a header:

• Follow the instructions on step 16 of the handout to insert the html components that will give the students a hard copy of their answers.
• Save using the name you already used for the confirmation page (save as  warmupconfirm.htm)

Once you have loaded these pages onto your web server, they will become 'live'.
